Palliative/End of Life Care
We understand how difficult it can be to see how a life limiting or terminal illness is affecting somebody you love. Support at Home can provide specialist support so your family can make the most of your time together.
We believe that home really is where the heart is and genuinely caring for the customers we support is what makes us such a passionate and supportive team.
During this difficult and challenging time, lots of people choose to spend their last moments in their own home, surrounded by personal items and the people they love. Having palliative care at home means your family member or friend can remain in the place they feel most comfortable. With the one-to-one support of a caring, compassionate and professional Support Worker, who is trained and experienced in palliative care, you can be reassured that your loved one will receive high quality, personalised care.
Our specialist Support Workers receive extensive and ongoing training in End of Life/ Palliative and Dementia Care.
Dementia/Alzheimer's Care
Continuity of care and a familiar face is one the most valued aspects of care when choosing a provider to support a loved one who is experiencing memory difficulties or who has been diagnosed with Alzheimer's or Dementia.
Our care service is designed to ensure that a personalised, bespoke plan of care is provided. We work with you to ensure that the right Support Workers are selected so that trusting, supportive relationships can develop, making everyday routines and tasks more manageable.
We can provide support with the many challenges that families face when a loved one is living with this condition, ensuring that you can make the most of spending time with your loved one.
Respite Care
At times, the family member caring for a loved one can benefit from a short break, away from the situation to rest and relax. We offer short-term respite care to relieve family members of care duties from time to time. Our personalised services ensure that the individual can remain comfortable in their own home, while receiving the care and support they need.
